Modified let statement to reach through relationship to grab OT details. Added in captures for Forms, Referral and Clients table which were referred to in existing scripting. Added State to OT Practitioners table view layout. Populated address data using existing Block information for OTs. Created test OT. Tested creating Referral email with "LittleMan Test" OT. Email populated with expected OT information. Confirmed OT detail modifications eg phone number change was reflected when creating a new email.
